Modoc Nation Wellness Center is open to Modoc Members and to all Modoc Nation employees.

Membership access is at no cost to you, your spouse/partner, and children under the age of 18 who reside in the home.

Please complete the membership form and email Lexus Williams to schedule an appointment time to do a tour of the Wellness Center and provide you with your access code / key fob for entry.
